Missed Appointment Policy
Currently there is no charge for a missed appointment, however 24 hours cancellation notice would be appreciated.
Patients must be seen by the family physician before consideration will be made for a referral to a specialist.
What to Bring to Each Visit
Please bring all current medications in their original bottles or an up to date list of all current medications (name, dosage and frequency of administration) with you. You also need to bring your Health Card to all appointments.
Not all services are covered by OHIP and as such you will be notified when a payment is required for such services. Non-payment after a reasonable time frame will result in discontinuation of uninsured services for that particular patient.
Prescription renewals for medications for stable medical conditions will be renewed over the telephone. The medication name, dosage, prescription number and pharmacy are required before a prescription can be renewed. Narcotic prescriptions will not be renewed over the telephone and patients are required to book an appointment to have these medications renewed.
Dr. Lunn has the pleasure of working with Nurse Practitioner Melissa Forgrave, to assist with the care of her patients.
